What Is All-on-4®?
All-on-4® is a technique developed by Professor Paulo Malo for Nobel BioCare, a swiss dental implant company. ‘All-on-4®’ refers to a full arch of teeth attached to four dental implants.
Treatment involves placing two implants at the front of the mouth and two implants at the back of the mouth at a 45-degree angle, carefully avoiding nerves and sinuses.
New teeth are attached within 24 hours following surgery restoring the smile quickly and efficiently.

1. Consultation and 3D Smile Planning
We take the time to understand your treatment goals and guide you through the treatment process.
We provide a full examination, advanced 3D diagnostics and facial scanning, to assess your jawbone and determine your suitability, and a personalised plan with an outline of costs.
Following this appointment, we take pre-surgical records and you meet with our laboratory technician to select your preferred tooth shape and shade to suit your facial features and smile.

2. Implant Placement & Immediate Teeth
Your dentist begins by removing any remaining teeth and precisely places between 4 to 6 dental implants into your jawbone.
Your dentist then carefully closes the gums and takes digital impressions to record the position of your implants, allowing our in-house lab technician to custom-design your bridge.
If your treatment plan includes bone grafting or pterygoid implants, this is done at the same time.

Within 24 hours, we fit your custom acrylic bridge, crafted in our onsite dental lab from high-quality, durable acrylic.
Our onsite lab allows us to maintain full control over the quality, fit and aesthetics of the bridge to provide you with strong, natural-looking teeth while you heal.
3. Healing & Guided Reviews
Over the next 3–6 months, your implants gradually integrate with your jawbone through a natural process called osseointegration.

During this time, we’ll see you for a series of scheduled review appointments to closely monitor healing, check the stability of your implants, and ensure your temporary teeth remain comfortable and functional.
We’ll also provide personalised guidance on oral hygiene, including how to care for and clean your bridge with your new Water flosser.
4. Crafting Your Long-term Teeth
With every full-arch implant treatment, a temporary acrylic bridge is fitted within 24 hours of surgery—so you can leave with a functional, natural-looking smile while your implants heal. This bridge is designed to be worn while your mouth heals, but may last up to two years with proper care.

Temporary acrylic bridges are covered by a 12-month warranty. Acrylic teeth can be replaced with a zirconia bridge any time after three months from surgery or worn for up to two years if properly cared for.

What is your All-on-4 payment policy?
We ask for a deposit of $5000. This allows us to to secure a surgery date for you and begin ordering implant components. The final balance is due 3 weeks prior to the surgery date. You are welcome to pay in full, if preferred however it is not necessary. Treatment costs cover:

- Post-operative appointments
If you need to cancel your surgery, payment is refunded. If you cancel within 2 weeks of the surgery date, a $500 admin fee applies to cover work and planning to date. Please speak with your treatment coordinator for questions about payment or finance options.

How much training does a general dentist need before placing implants?
When it comes to performing dental implant surgery, there are currently no mandated requirements governing the amount of training a general dentist must complete before placing dental implants. In some cases, a dentist may undertake a short 1–2 day course and begin placing implants with limited experience. However, research indicates that clinical experience plays a significant role in treatment outcomes.
Dr David Norcross and Dr David O'Dowling have each undertaken extensive postgraduate education in implant dentistry, completing advanced programs and clinical residencies over many years.
Dr David Norcross graduated from Leeds University with a Bachelor of Dental Surgery. He went on to complete a one-year Master Clinician Program in Implant Dentistry through the Global Institute for Dental Education (gIDE) in collaboration with the University of California, Los Angeles (UCLA). He has also completed clinical residencies at the Malo Clinic in Melbourne and Portugal — the birthplace of the All-on-4® technique — gaining advanced training in full-arch rehabilitation.
Dr David O'Dowling graduated from University College Cork in Ireland and completed a Master of Science in Oral Implantology at Goethe Dental School in Germany. What is All-on-4®?
All-on-4® is a full-arch dental implant treatment used to replace all missing or failing teeth in the upper or lower jaw. Rather than replacing each tooth individually, a complete bridge of teeth is supported by four strategically placed dental implants.
The technique was developed by Nobel Biocare and involves placing two implants towards the front of the jaw and two implants towards the back at an angle. This approach can often maximise the use of available bone and may reduce or eliminate the need for bone grafting in suitable patients.
All-on-4® has become one of the most widely used full-mouth dental implant solutions worldwide and has demonstrated high long-term success rates of up to 98% in clinical studies*.
The term All-on-4® is a registered trademark of Nobel Biocare and specifically refers to the treatment concept when used with Nobel Biocare implants.
Today, many clinicians use similar treatment protocols with four, five, six or more implants depending on the patient's anatomy, bone quality and treatment goals. As a result, you may also hear terms such as All-on-X, which refers more broadly to full-arch dental implant treatment supported by a customised number of implants.
For patients requiring full-mouth rehabilitation, the number of implants used is less important than selecting the treatment approach best suited to their individual needs.

Why do All-on-4® dental implant costs differ so much?
All-on-4® costs vary because of several factors including the skill, training and experience of the dentist placing your implants; and person designing your new teeth; the quality of teeth and dental implants; and treatment plan inclusions.
1. Skill, training and experience of the dentist
General dentists Dr David Norcross and Dr David O'Dowling have restricted their practice to restoring smiles with dental implants and have completed extensive advanced training in implant and restorative dentistry. Both dentists also lead clinical education courses in advanced implant dentistry, helping dentists further develop their skills and expertise in implant treatment.
2. Skill, training and experience of the dental technician
All of our full mouth implant bridges are custom-designed by highly experienced dental technicians. Perth Dental Implant Centre uses a local lab and onsite lab to design full mouth bridges that are high in quality, durability and aesthetics that complement your face, age and personality.
3. The type and quality of replacement teeth
Our dental implant packages offer a number of teeth replacement options and materials to suit your aesthetic and financial needs. Replacement teeth materials have different strengths, durability, aesthetic features and longevity.
4. Type and quality of dental implants
Perth Dental Implant Centre can place any implant system you choose. However, for full arch replacement we offer pricing packages using two high quality implant systems: Nobel BioCare and Southern Implants.
5. Treatment plan inclusions
All treatment plans include clinical radiographs and review appointments at two and four weeks after treatment. If you choose to upgrade your teeth after 3 to 4 months of your mouth healing post-surgery, we’ll include this item in your treatment plan.

Why do I need an initial custom-milled PMMA (acrylic) bridge? Can’t I just get a zirconia bridge?
Zirconia bridges are milled from solid blocks of pure zirconia, a high-quality tooth material that’s known for its strength and aesthetics. Zirconia bridges take between 2 to 4 weeks to manufacture.
In the week following dental implant surgery, the jaw bone begins to soften around the implants, starting a healing process known as osseointegration. A tooth bridge needs to be attached to implants in the first 7 days of surgery, while the bone is still rigid enough to hold it in place. This is why an custom-milled PMMA (acrylic) bridge is placed first.
Once jaw bone and dental implants properly fuse together over 3 to 4 months, new impressions can be taken to design a long-term zirconia bridge. Otherwise there is no rush as your custom-milled PMMA (acrylic) bridge can last up to 2 years on average with proper care.
When it’s time to replace your bridge, you can choose from another custom-milled PMMA (acrylic) bridge at $2,000 per arch or $3,500 for both, or a long-term zirconia bridge, which may last between 10 to 15 years on average, or longer. Zirconia bridges start from $10,500 per arch.
